2008 Girls Fun with Fundamentals Winter Camp Registration Form
The winter camp is for girls in 3rd through 6th grades. It will be held in the Milton High School gym on December 22-23 from 12:30PM-3:00PM. Please regeister by December 19, 2008. Conact Coach Randall at 868.9300 x1055 or 868.4767 with questions.

Tri-County Player Waiver Form
The Tri-County player waiver form must be signed by the parent and child participating in the Tri-County league. This form should be kept by the coach of the team.

Tri-County Coaches Waiver Form
The Tri-County coaches waiver form must be filled out and signed by each coach of a Tri-County team. This includes assistant coaches. This form must be returned to Jeff Ezdon.

Tri-County Team Registration Form
The Tri-County team registration form must be filled out by the coach of each Tri-County team. This form needs to be returned to Jeff Ezdon.

Tri-County Policies & Procedures

MYB Coaching Application
All Milton Youth Basketball coaches are required to fill this application each year. It may be turned in at registration or mailed to "Milton Youth Basketball" - PO Box 241, Milton, WI 53563.

MYB 2008 Player Registration Form
This form is used to register a player in Milton Youth Basketball. The form can be mailed or brought to registration. Registration for the MYB 2008/2009 season will be held August 3rd and 24th at the Milton Middle School from 4:00PM-6:00PM.

MYB 2008 PDT Registration Form
The MYB PDT registration form is used to register pre-determined teams. The form must be filled out and returned and brought to registration at the Middle School on Augst 3rd or 24th from 4:00PM-6:00PM. Deadline for PDT registration is August 24th.

Milton Youth Basketball Complaint Form
Use this form to submit a written formal complaint to the MYB board. Only use this form if your grade representative can't resolve your issue.

2008-2009 MYB Handbook
The Milton Youth Basketball board has assembled this manual in an effort to answer most of your questions about the Milton Youth Basketball program.

MYB Equipment Request Form
Coaches should use this form to request equipment at the begining of the season. Equipment includes game jerseys, practice jerseys, coaches bag, basketballs, medical kit and score books.The form should be returned to the Equipment Manager.
